Admission Procedures

Horizons Academy accepts applications from students five through 20 years of age who have a diagnosis of a specific learning disability, or recognized learning disorder.  Diagnoses will be accepted from private professionals, diagnostic and testing centers, and the Department of Education/Department of Health.  It is understood that many of the disorders that we accept are accompanied by behavioral difficulties.  However, we do not accept children who have serious emotional or behavioral concerns.

Horizons admits students based on the following required information:

a)     The Horizons Admission application form,

b)    testing and educational records,

c)     a parent interview, and

d)    a day visit by the child

As part of the application process, we require copies of cognitive testing, educational testing, and the child’s latest Individualized Educational Program (IEP), if available.  If the application has received mental health services and/or has a mental health diagnosis such as attention deficit disorder, anxiety disorder, etc., mental health records/evaluation should be included in the application packet.  A one-time, non-refundable application fee of $50 is required with all applications.  If the child is determined to meet admission criteria, the child will be added to the list for enrollment.  There may at times be a waiting list.

Openings will be filled from the waiting list according to the youth’s age, skill level and the length of time that they have been on the waiting list.  Available openings will be determined by the number of children who transition out of the program at the end of each school year or by those children who may leave the program during the year to attend a different school.

Upon acceptance into Horizons, the parent will receive verbal and/or written notification of acceptance along with a registration form.  The registration form must be completed and returned with a non-refundable deposit of $100 to the school by the specified deadline to secure the child’s spot.

For students that are placed at Horizons Academy by the Hawaii State Department of Education all students must be accepted via the standard admissions procedure before they will be considered as Department of Education placements.
